When a project fails, conducting a retrospective is a good practice, which typically includes three questions:

  1. “What went well?”
  2. “What could have gone better?”
  3. “What we should we differently next time?”

Jacob Kaplan-Moss suggests adding a fourth question, which should be asked when the project is successful:

  1. "Where did we get lucky? What could or should have failed, but didn't?"
